I authorize Culinary Facilities of Georgia, LLC (dba "PREP®") to charge my payment method herein on a recurring monthly basis for an annual Food Truck Commissary / Central Preparation Facility ("CPF") contract, which shall have an effective date of today and a termination date 365 days from today. The fees are non-refundable, and in the event of earlier termination I agree to allow PREP® to charge my payment method for the balance of the remaining annual contract. Unless I otherwise notify PREP® via email (with confirmed receipt) of my intent to cancel, I hereby authorize PREP® to continue to charge my payment method monthly for CPF contract renewal fees at the same monthly rate upon contract renewal which occurs annually 365 from today. I acknowledge that in the event of cancellation of my contract, PREP shall notify the respective Public Health or Regulatory Body of such cancellation, and I shall be required to contract with another approved Food Truck Commissary / Central Preparation Facility in order to maintain compliance with local regulations regarding mobile food vendors.
